Application & Documentation

The Financial Assistance Membership application is available online at the link below or at all YMCA branch Welcome Centers. The household income verification can be done at the time of application if documentation is provided.

Please be ready to show:

  • Two months of employment check stubs from all working adults in the household.

  • Your most recent tax return statement.

If those documents are not available or applicable, the Y will also accept paperwork documenting income assistance from government or charitable providers to verify your income need. A list of possible sources of assistance can be found on the application form.

If you would prefer a private setting to discuss your financial need or if you need help with completing any part of the application you may make an appointment with a Y staff member in your local branch.
